Have you experienced loss of smell or taste in the last 7 days? - YES/NO

Have you been in contact with anyone suffering the above symptoms in the last 7 days? - YES/NO

Do you belong to a high-risk group? - YES/NO

We would like to reassure you that we have introduced stringent measures to minimise the spread of infection but there is always a risk.
